At Forest Children’s Centre, we encourage full use of the Centre by children, adults, parents, carers and outside agencies. We aim to serve the community fully.

Many of our services and facilities require you to fill out a registration or application form which are available from the main Centre reception.  The forms are also available for you to download from this website. The forms are in Adobe PDF format (find out more about this format here).
